One of the goals that we have as dentists is treating our patients’ condition while saving as much of their natural tooth structure as possible. We follow the philosophy of minimally invasive dentistry.
When we think about restoring teeth with extensive occlusal wear in the anterior, we immediately think about doing full coverage crowns and voila, issue has been fixed! But has it really been successfully treated long term? Have you ever thought about treating a case of severe anterior wear with veneers using the tried and true PVS impression?
I didn’t think it was very practical until I had an opportunity to talk to Dr. Jose-Luiz Ruiz. Dr. Ruiz is the Director of the Los Angeles Institute of Clinical Dentistry and is a strong proponent of minimally invasive dentistry.
Dr. Ruiz discusses many of the benefits of supra-gingival minimally invasive dentistry for treating severe occlusal wear and it was truly an eye opening lesson for me. By combining the three golden rules for occlusion and utilizing adhesive dentistry, most dental cases can be successfully treated. Not only that but the benefits of reduced inflammation on your entire body is astounding when viewed holistically.
With the growth of digital scans and milling machines, there have been discussions that traditional impression materials are antiquated and you can chuck those out of your drawers. That might not be true (at least for now).
One of the issues that we have to consider is how your preps must be adjusted when working with digital scans. Most of the time, more tooth structure has to be removed to accommodate the restorative material. Is sound tooth structure being removed unnecessarily in the name of technology?
How about complex cases where you are trying to scan multiple cases? Dr. Ruiz talks about these type of cases and how traditional PVS impressions are much easier for the labs to work with and the results are life changing in many ways.
Digital scans are here to stay but in a number of cases, they are not yet at the level that a good quality PVS impression can offer.
